Here’s an important reminder that if you are one of the 128 Illinois public libraries receiving a Bill and Melinda Gates Foundation Opportunity Online Hardware Grant, you must complete an important survey that is a requirement for all participants in the program. Please complete the survey no later than October 31, 2009.

When you have completed the survey, you must trigger the verification process by clicking on the Verify Survey Completion button located on the Instructions page. If your survey is complete, you will see four green checkmarks. If you survey is not complete, you will see a red exclamation point. This indicates that all of the required questions have not been answered and will have to be completed before the survey can become finalized.

The survey is administered by the Center for Library & Information Innovation at the University of Maryland-College Park, and technical assistance is available. The study is funded by the Bill & Melinda Gates Foundation and the American Library Association. Contact Mark Shaffer if you have questions.
